APPOINTMENTS

Appointments for passports are available, but are not required.

The office of the Clerk of the Circuit Court is a designated passport Application Acceptance Facility. We process passport applications for U.S. citizens only and forward them to Passport Services for processing.

RENEWALS

IMPORTANT: Passport renewals are required to be submitted by mail directly to the Department of State (IF YOU) Meet the following requirements:

• You possess a passport that is not more than 5 years past the expiration date, And
• You were at least 16 years of age when it was issued

If you qualify, complete form DS-82 and return to the address listed on the form.

For your renewal you are required to mail the following:

• Completed renewal form, DS-82 obtained from the Clerk’s office or website travel.state.gov
• Check or money order made out to the U.S. Department of State for $130 per application;
• 2 identical photographs;
• Check or Money order to U.S. Department of State – see fees above
• Your expired passport.
• Name Change by Marriage or by Court Order

REQUIREMENTS FOR AN APPLICATION 

Proof of citizenship
Social Security Number

AND

• Certified birth certificate with a raised seal and file date and a copy of birth certificate. Minors under 16 years and renewing your passport, you need both documents (Certified birth certificate and prior passport)
  
• Prior passport
• Completed form DS-11 (Click here to upload form)

OR

• Original naturalization papers;
• 2 identical photographs – No eye glasses in photo. Photos are taken at UPS, AAA, Walgreens or CVS.

Identification such as a valid driver’s license (Not Temporary OR Learner’s Permit), State ID, valid passport with the signature and photograph of the applicant.

• Passport Book 16 Years of age and older * ($130.00)
• Passport Card 16 Years of age and older* ($30.00)
• Passport Book 15 Years of age and younger * ($100.00)
• Passport Card 15 Years of age and older *($15.00)

Clerk’s Fee

• Cash, Credit/Debit Card – Clerk’s Fee per Application * ($35.00)
• Credit / Debit Card Fee *3.5%
